Kitsap Mental Health Services is hiring a
Re-Entry and Behavioral Health Court Clinical Care Coordinator. This position will be pivotal in ensuring individuals transitioning from the Department of Corrections and participating in Behavioral Health Court receive the services and support they need. You will provide comprehensive case management and therapeutic interventions to help clients achieve stability, access essential resources, and thrive in their communities. Primary Responsibilities:
- Oversee the continuum of care for Re-entry Community Safety Program (RCSP) clients, including pre-release engagement, Multi-System Care Planning Team (MSCPT) coordination, and post-release intensive services.
- Serve as the primary care coordinator for clients in Behavioral Health Court (BHC), ensuring clinically appropriate services are delivered based on individual needs.
- Develop, implement, and evaluate personalized treatment goals and plans in collaboration with a multidisciplinary treatment team.
- Participate in weekly Behavioral Health Court hearings and meetings to support client progress.
- Educate and support clients' families while advocating for clients' rights and preferences. Consult with community agencies, including DSHS, housing authorities, and court systems, to facilitate care coordination.
- Provide crisis intervention and collaborate with Designated Crisis Responders when necessary.
- Monitor clients for Less Restrictive Alternative (LRA) compliance, facilitate extensions, and assist in determining when revocations are appropriate.
- Engage with clients post-release to encourage continuation of care and services as needed beyond the RCSP program's maximum benefit period.
Minimum Qualifications: EDUCATION: Bachelors Degree in Psychology, social services, or behavioral health field EXPERIENCE: Entry level (no prior related work experience) This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must meet agency driving requirements, including meeting the minimum age requirement of 21 years old, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver's license in the state of Washington, and having reliable, insured transportation.LICENSURE: Agency Affiliated Counselor Registration (can be obtained after hire)
Preferred Qualifications: EXPERIENCE: Experienced (minimum 2 years of job-related experience) Experience working with behavioral health consumers in a clinically oriented setting. Experience entering data into an electronic medical record.
