Required Qualifications
  • Bachelor's degree in Social Work, Psychology, Sociology, Public Health, Nursing, or a related field.
  • A minimum of one (1) year of experience working with patient populations receiving medical care or social services.
Preferred Qualifications
  • Experience in care coordination and/or case management.
  • Experience with health IT systems and reports.
