About the Role:We are seeking a dedicated Care Coordinator to join our Medication Assistance Program/Outpatient Services team. This position will provide basic case management and clinical services to women in Ohio struggling with substance abuse and mental illness. The Care Coordinator will play a critical role in facilitating successful treatment outcomes through direct support, collaboration, and connection to essential resources.Hours: 5:30 am to 2:30 pmMinimum Qualifications:
  • Bachelor's degree in nursing or related field
  • Current state nursing license
  • Minimum of 2 years of experience in care coordination or related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
Preferred Qualifications:
  • Associates Degree or higher in psychology, social work, or a related field (Master's degree preferred).
  • Certified Chemical Dependency Counselor Assistant (CDCA) certification in the State of Ohio.
  • Must have a valid Ohio driver's license, reliable transportation, and current insurance.
  • Minimum of two years experience working with individuals diagnosed with mental illness or related experience; trauma training is a plus.
  • Strong written and verbal communication skills, with the ability to engage effectively with clients, colleagues, and community partners.
  • Experience with DSM IV diagnoses, evidence-based practices such as motivational interviewing, and trauma-informed care.
  • Ability to work both independently and as part of a collaborative team.
  • Excellent organizational and time management skills.
  • Experience working with electronic medical records
Responsibilities:
  • Provide therapeutic and case management services to adults with substance abuse and improving their functioning in the community.
  • Conduct outreach and engage potential clients in jails, hospitals, and street settings.
  • Perform comprehensive assessments to evaluate diagnoses, levels of functioning, and support networks.
  • Assist clients in setting goals and developing coordinated care plans, ensuring annual updates.
  • Ensure clients apply for and receive necessary benefits, including SSI, SSDI, Medicaid, and Medicare.
  • Facilitate access to local resources, such as medical care, housing, rehabilitation programs, and social services.
  • Engage with clients who have not maintained contact, ensuring they remain connected to services.
  • Deliver therapeutic interventions, including individual, group, and crisis counseling.
  • Maintain accurate documentation in the Electronic Health Record (EHR) system.
  • Meet weekly, monthly, and quarterly billing targets.
  • Collaborate with team members, participate in 24-hour on-call coverage, and assist with daily operations of related programs.
Skills:As a Care Coordinator, you will need to have strong communication and interpersonal skills to effectively coordinate patient care plans and work with medical staff. You will also need to be highly organized and detail-oriented to ensure that patient care is well-coordinated and that all necessary resources are available. Additionally, you will need to have strong critical thinking and problem-solving skills to address any issues that may arise during patient care. Finally, you will need to be proficient in the use of electronic medical records and other healthcare technologies to effectively manage patient care.Work Environment & Physical RequirementsThis position requires regular use of hands, fingers, and the ability to communicate effectively. The Care Coordinator will frequently be required to walk, balance, stoop, kneel, and drive. Occasionally lifting up to 15 pounds is necessary. The work may include driving to various locations and providing transportation for clients.
