Summary:Maximizing the utilization of participant medical records, accessed via the West Virginia Health Information Network (WVHIN) web-based portal, and a document sharing portal with electronic signature interface for the goal of streamlining the WIC certification process and enhancing continuity of care for WIC participants.Position Responsibilities:* Secure WVHIN acknowledgment form for all applicants and participants within the assigned local agency service area prior to certification appointments; Upon receiving online application or scheduling of new applicant appointment, send participant the WVHIN acknowledgement via email attachment or link via text to receive participant authorization to search WVHIN records.* Streamline WIC applicant and participant certification by gathering anthropometric measures and immunizations from medical records accessible, and authorized by participant, through the WVHIN. Pull participant schedule for each clinic within the assigned local agency area; search for each participant within the WVHIN; enter data collected by a health care provider within the most recent 60 days and save information in the Anthro/Lab Crossroads MIS screen; enter family alert to notify staff if Anthro and immunization status were completed for the certification prior to appointment.* Serve as interface/conduit for document sharing portal and Crossroads MIS by ensuring all documents are submitted and routed appropriately as well as included in the participant record.* Focus on promoting the WIC Program and public health nutrition services in health care environments. Meet with providers and/or office managers to assess needs, evaluate what is working, and establish and maintain rapport for positive collaboration. Supply WIC materials order form, prescription pad, WIC-53, and outreach materials.* Work with state and local WIC Outreach Coordinator to understand WIC services, eligibility, and certification process.* Define outreach goals and priorities as well as develop implementation action plans for each assigned county within the Local Agency through analysis of WVHIN enrollment and recruitment strategies.* Use multiple communication strategies with PCPs and WIC participants to convey coordinated messages and materials related to WIC services and streamlined certification methods accomplished through medical partnerships; Serve as the primary connection between Local WIC Agency and health care providers with focus of promoting WVHIN participation to increase the number of accessible records, and data sharing with Local WIC Agency.* Track, analyze, and present WVHIN information and data.* Track and maintain record of all activities and contacts as required by grant reporting and to measure outcomes.* Strengthen WIC as an important adjunct to the medical home and represent appropriate adjunct health support to prenatal, pregnant, perinatal, infant, and child primary care.* Cultivate and sustain strong, reciprocal relationships between medical schools and health related programs to assist in developing and implementing long and short-range partnership plans, future collaborations, and WIC Grant projects.* Assignment to include geographic area or region; travel required.Qualifications:Training: Graduation from an accredited four-year college or university.Substitution: Full-time or equivalent part-time paid experience in paraprofessional or responsible clerical work at a level consistent with the work performed by an Office Assistant III and which required application of state and federal laws and/or guidelines, may be substituted for the required training on a year-for-year-basis.Valid Driver License.Prior WIC experience or understanding of WVHIN preferred.Skills and Abilities:To perform this job successfully, an individual must be able to perform each essential duty.
