You'll coordinate patient screenings for specific risk factors, guide care coordination for positive results, and drive the EMPOWERED program intake and CARA Plan of Care processes. Beyond patient-facing responsibilities, you'll keep the office running smoothly by managing inventory, ordering supplies, maintaining the program calendar, and handling diverse administrative tasks. Reporting to the program director, you'll play a pivotal part in enhancing both patient outcomes and operational excellence.Essential Functions:
- Patient Screening and Care Coordination: Collaborates with Carson Tahoe MOMs Clinic staff to screen individuals at risk for substance use disorders and initiate CARA Plans of Care for eligible patients.
- Referral and Intake Management: Screens, schedules, and coordinates intake for new referrals while tracking attendance for patients and visitors.
- Administrative Support: Oversees daily operations, stocks materials, maintains inventory, and monitors office equipment, including submitting IT helpdesk requests as needed.
- Volunteer Oversight: Provides supervision and guidance to program volunteers, ensuring smooth support for various activities.
- Event and Facility Coordination: Manages service requests for event setup and assists with organizing special events such as sober social activities and health education classes.
- Flexible Responsibilities: Participates in additional duties as assigned to support the EMPOWERED office and its programs.
Qualifications:
- Bachelor's degree required.
- A minimum of three years of experience working in behavioral health.
