Shawnee Health - Care CoordinatorJoin Southern Illinois' leading healthcare organization, with over 380 team members who believe that in taking care of each other and our patients and clients, we create new opportunities for success and bring big dreams to life.Shawnee Health is seeking a professional Care Coordinator for our Community Care Program covering Saline and Gallatin Counties in Southern Illinois with the option for remote work. This position reports directly to the Care Coordinator Supervisor.Responsibilities* Determines eligibility and creates care plans for Illinois Department on Aging* Performs assessments and/or reassessments of senior citizens within their own homes, area hospitals, or long term care facilities as authorized by SHSDC and in accordance with the approved policies and procedures* Determines, coordinates, and develops written care plans related to results of the assessment* Coordinates services for older adults to remain independent* Educates on resources, options and provide case management to older adults and their families* Assists and completes Medicaid applications and documentation of DPA eligibility for appropriate CCC clients under the guidelines and policies for IDOA and IDPA.Requirements* Requires a BA/BS in the human service field with emphasis on social work, rehabilitation counseling, or psychology preferred* Must have valid driver's license and transportationSalary Range -$43,888.00- $46,561Full-time employee benefits include:* 12 vacation days earned in the first year* 11 paid personal days/holidays per calendar year* 6 sick days earned per year* Retirement plan with 3% company contribution after becoming eligible in the plan (a matching contribution by the employee is not required to receive this benefit).
