We are also proud to provide competitive wages, generous paid time off, and several voluntary benefits like ambulance and disability insurance, among others.The Role You PlayAs a patient navigator here at Pawnee, you will be responsible for meeting with clients new to Pawnee's services - those with a severe mental illness, children with a severe emotional disorder, or adults with a co-occurring disorder - and orienting them to the variety of services available at Pawnee. You will stay with the clients until discharge and collect information from them at regular intervals to help track their outcomes.* The Day-to-Day* Meet with new clients in the target populations above and orient them to the array of services at PMHS.* Familiarize your clients with community resources available for educational, cultural, recreational, and social activities.* Perform care coordination - assisting the client with accessing physical health care, social services, housing resources, and employment resources to facilitate whole person wellness and recovery.* Other Duties as Assigned* Perform case management duties as necessary, including goal development and individual therapeutic intervention.* Driving clients to appointments, taking them to the grocery store, pharmacy, etc., meeting them in public settings or in their home.* Perform follow up when a client is referred to a provider outside of Pawnee to ensure services were provided.Who You AreThe ideal candidate is friendly and well-organized - you will need to utilize both customer service and patient care skills to ensure that your clients feel supported throughout their experiences with Pawnee. You will also need to be patient and an excellent communicator, both with clients and with Pawnee providers and staff.Required:* Bachelor's degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education.* Strong oral and written communication skills using the English language.* Basic personal computer skills which enable the entry, retrieval, and use of electronic data.* Experience with Microsoft Office software.* A valid driver's license and automobile liability insurance maintained in accordance with Pawnee policy and procedure.* Ability to work with a wide variety of individuals with behavioral health needs served by the Community Mental Health Center.* Excellent organizational skills.* Ability to effectively multi-task.* Case Management experience.Preferred:* Knowledge of the needs and challenges of adults who experience severe and persistent mental illness and/or children with a severe emotional disturbance.* Experience working with the BIPOC or Veteran community.Results of criminal and all other applicable background checks must meet the approval of Pawnee Mental Health Services.Pawnee Mental Health is an Equal Opportunity Employer, and all applicants will receive equal and equitable consideration without regard to race, religion, color, gender, sexual orientation, age, national origin, disability, or any other status or condition protected by law.
