Social Worker - Holloway Community Program, 8:30a-5:00p

PRIMARY FUNCTION:

The Social Worker works as a team member of the William J. Holloway Community Program. This position addresses potential barriers to care by facilitating and coordinating connection to available resources to ensure patients can access care including Ryan White and ADAP funding for patients infected with HIV. This position will assist patients and their families in adjusting to personal and environmental difficulties which predispose illness or interfere with medical care as it relates to the patient's HIV infection. This position is for the William J. Holloway Community Program- Wilmington Gateway site.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Participates as a member of the practice care team, focused on addressing financial barriers to care, care coordination, and connecting to available community resources to address social care needs of patients.

Maintains a current knowledge base of HIV disease process. Utilizes knowledge to appropriately assess patient needs and implements or facilitates appropriate intervention.

Acts as a resource person and assists in educational efforts involving HIV, with particular emphasis on psychosocial factors.

Conducts financial screening for Ryan White, ADAP and ADAP co-pay services in keeping with established organizational and Division of Public Health policies. Assists eligible patients in enrollment into Delaware Medicaid or private insurance through the ACA Marketplace. Utilizes MAGI process to determine and document current FPL level. Assists patients with applying for ChristianaCare Financial Assistance and HCC/SFL

Evaluates and provides psychosocial support to patients and their families/significant others.

Assists patients in accessing appropriate supportive services in the community.

Maintains current knowledge of resources available to assist other health care providers and HIV program staff in coordination of care.

Collaborates with community case managers and community agencies to provide a comprehensive continuum of patient care and to maximize utilization of available resources.

Maintains pertinent and timely documentation in patient's medical charts and departmental records.

Demonstrates skills and knowledge necessary to provide care appropriate to adolescent, adult and geriatric patients, including knowledge of growth and development, the ability to obtain and interpret information to identify patient needs, and to provide the care needed.

Adheres to ChristianaCare policy regarding release of information, patient privacy and confidentiality, and discerns when PHI should be shared to best coordinate care.

Participates in departmental meetings, multidisciplinary team meetings, case reviews, and contributes to performance indicators for the program.

Assists in data collection, CAREWare database entry, maintenance of required patient records, and quality assurance monitors when appropriate.

Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.

Represents Christiana Care and the William J Holloway Community Program in a positive, professional manner.

Obtains requisite CEU's for license renewal as well as attends mandatory education programs necessary to provide expert client care, regardless of whether or not CEU's are offered.

Performs other related duties as required.

EDUCATION AND EXPERIENCE REQUIREMENTS:

Bachelor's degree in social work or related field from an accredited college/university required.

Master's degree in social work (MSW) from an accredited graduate school of Social Work preferred.

Licensed BSW or MSW as required by the state of Delaware.

Demonstrated interest in HIV patient management.

Bilingual language skills are a plus.
