Job Title: Hospice Registered Nurse Case Manager
Location: Kingston, NY
Salary: $80,000 - $90,000 per year
Summary: We are seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to provide high-quality care to patients and their families during end-of-life transitions. The ideal candidate will coordinate patient care, ensuring comfort and dignity.
Responsibilities:
- Assess patient needs and develop individualized care plans
- Provide direct nursing care and support to patients
- Collaborate with interdisciplinary teams for optimal care
- Educate patients and families on hospice services
- Document patient progress and maintain accurate records
Requirements:
- Current RN license in NY
- Minimum 2 years nursing experience, preferably in hospice or palliative care
- Strong communication and organizational skills
