Together, we are committed to breaking the cycle of homelessness and helping our clients achieve long-term self-sufficiency.Position Summary: The Outreach Specialist will work throughout San Mateo County in Region 3, covering the following areas: Belmont, Foster City, Hillsborough, Redwood City, San Carlos, and San Mateo, identifying unsheltered unhoused people residing in encampments and other places not meant for human habitation. The Outreach Specialist will utilize a multidisciplinary approach to engage the hardest to serve homeless in an effort to connect them to services. Responsibilities include providing outreach, engagement and rapid response services to unsheltered individuals, make appropriate referrals, connect clients to social services, and keep up-to date case files.Hours: M-F, 8am-4:30pm; May be occasional weekend or after-hours on-call work.Outreach Specialist Duties:
- Spend approximately 70% of time in the field, providing direct services to unsheltered homeless individuals, including CES assessments, shelter placement, and rapport building.
- Act as the first point of contact for partner or team referrals for homeless services, and serve as the primary contact for rapid responses, outreach requests, and encampment removal coverage, ensuring prompt responses within contractual timeframes.
- Identify the target population, perform screening assessments, and provide referrals and linkage to primary care, mental health services, short-term residential stabilization, and other social services.
- Collaborate with Case Managers to identify individuals who would benefit from intensive case management and provide comprehensive support.
- Engage clients using trauma-informed, harm-reduction services, supporting access to shelters, health services, and housing resources without prerequisites such as sobriety or income.
- Participate in monthly multidisciplinary team (MDT) meetings to collaborate on housing-focused case plans, progress, barriers, and resource needs.
- Monitor client progress and maintain accurate documentation and records in compliance with applicable standards, regulations, and grant requirements.
- Communicate effectively with stakeholders, including probation officers, social workers, and family members, while maintaining confidentiality and professionalism.
- Participate in continuing education activities to stay informed and knowledgeable in relevant areas of expertise.
- Perform other duties as assigned by the supervisor.
Qualifications
- Bachelor's degree in social work, psychology or related field, with a minimum of two years' case management experience
- Excellent communication and interpersonal skills; proven ability to collaborate with community partners
- Understanding of the issues faced by homeless individuals and other marginalized populations in crisis
- Availability to work occasional evenings and weekends.
- Experience working with mentally ill and dually diagnosed clients highly desirable.
- An understanding of ideas and practices that promote diversity, equity and inclusion; and a commitment to ongoing professional development in this area.
- Bi-lingual Spanish language capabilities a plus.
