Today, our clinics in Bridgeport, Danbury, Norwalk and Stamford are the primary source of free medical care for the uninsured in Fairfield County.

About the position: The Referral Coordinator is responsible for all referral processing and post visit reporting as well as resolution of any invoicing received for referrals initiated from Americares Free Clinic of Bridgeport.
Key Outcomes: In the first 90 days, the Referral Coordinator will:
- Learn and live the Americares values.
- Complete all require new hire onboarding trainings.
- Develop understanding of the free clinic model
- Complete Learning Modules for Athena
- Develop understanding of Quest and EPIC
- Begin to understand clinic policies and procedures
- Properly answer phones and manage calls
- Schedule AFC appointment and enter demographic information
- Schedule Diagnostic Appointments
- Schedule Specialty Appointments when applicable
- Update and Manage Referral Log
- Understand referral processes and appropriately submit referrals
- Demonstrate medical interpreting skills
- Retrieve Reports for visits and results from diagnostic testing
In the first 6 months, the Referral Coordinator will:
- Become skilled in EHR (Athena, EPIC, and Quest)
- Establish a connection with and increase existing referral sources
- Demonstrate consistent collection of social needs and successful referrals
- Incorporate the Unite Us referral platform software in referral process
- Master patient scheduling and referrals
- Master Referral Log
- Participate in case management
In the first year, the Referral Coordinator will:
- Be a fully functioning member of team with the knowledge and ability to assist team as needed
- Demonstrate ability to serve as a medical interpreter, as needed
- Perform projects as assigned by clinic director while maintaining Referral Coordinator responsibilities
Duties and Responsibilities:- Establish relationship with referral sources
- Schedule patient appointments for diagnostic testing and provide information to patients to ensure compliance with appointments
- Provide needed patient records to the referrals source and advise the patient of the details of the follow-up visit
- Develop knowledge of community resources needed by patients to help solve SDOH issues
- Establish collaborative and productive working relationships with a wide range of community organizations and social service agencies
- Accurately interpret conversations between health care provider and non-English-speaking patients following recommended guidelines.

- Familiarity with medical terminology
- Demonstrated organizational skills
- Ability to multitask and manage competing priorities to complete work in a timely and accurate way
- Demonstrated ability to work respectfully with a team, both following instructions and making suggestions for improvement
- High school diploma or equivalent
US work authorization is required for this role.
These competencies would be great to have, but are not required:- Fluency in languages that are spoken in at least one of our core locations (India, Philippines, Tanzania, El Salvador, Puerto Rico, Colombia, etc.), in addition to English and Spanish.
- Experience in a medical office setting
