Supervisor, Care Coordination at AllCare Health with the Population Health department in Grants Pass, OregonWe Are Seeking Qualified Candidates to Join Our Team!AllCare Health offers competitive wages, an excellent benefits package including affordable healthcare, 401k retirement, wellness programs, and flexible schedule options.SummaryThis position is responsible for providing supervisory oversight to the Population Health Care Coordination Team as they work with members receiving care from local agencies, hospital systems, and community partners and for providing program support to the Care Coordination Manager by performing the following duties:Supervises Population Health Care Coordination department staff.Provides direct services to members.Orients new staff and assures that training and education needs of staff are met.Assists members in understanding their health condition(s) and navigating the health and social service systems.Serves as subject matter expert in Care Coordination and Case Management interpreting regulatory requirements and coordinate the process to support those requirements.Works collaboratively with department leadership to identify and develop departmental objectives and strategies to maximize staff efficiencies and departmental operations.Assists with the development and annual review of policies, care programs, desk procedures, and standards for the department to ensure they meet CMS and State of Oregon Health Plan requirements.Supervisory ResponsibilitiesDirectly supervises members of the Care Coordination Team. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include participation in interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.All relevant experience can be listed in your resume to perform essential duties of the position including the following: lived, volunteer, professional, or a combination of experience and education.EducationBachelor's degree (BA) from a four-year college or university in a related field, required.Master's degree in a related field, preferred.ExperienceOne to three years of experience in program administration including grants management, community collaboration, supervision, and data management.Two to four years' experience working with underserved populations with demonstrated experience working with diverse populations in community settings.Certificates, Licenses and/or RegistrationsA Valid Oregon clinical licensure (RN, LCSW, RT, etc.)Valid Oregon driver's license and vehicle insurance.Current CCM (or willing to obtain within two years of employment).Current BLS Certification (or willing to obtain within 90 days of employment).Current Mental Health First Aid Certification (or willing to obtain within 90 days of employment).Physical Demands & Work EnvironmentThe employee must occasionally lift and/or move up to 10 pounds. While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel and talk or hear. The employee is occasionally required to stand; walk and reach with hands and arms.
