Function:The Family Nurse Practitioner (FNP) provides primary care services, focusing on health promotion, disease prevention, and acute and chronic illnesses management. FNPs work collaboratively with other providers and healthcare professionals to ensure high-quality patient care. This role is essential for promoting health and improving client outcomes in diverse populations.Organizational duties & responsibilities:
  • The primary responsibility of all staff is to ensure the safety and well-being of all Rame-Estep/Re-group (RE) clients.
  • Supports RE's mission, vision, and values. Facilitates and adheres to the agency and licensure code of ethics, policies, and procedures.
  • Supports all functions that attain and maintain accreditation and compliance with regulatory agencies.
  • Supports and facilitates positive interaction with clients, staff, families, and community partners by exhibiting the following traits in the office and in the public when carrying out job duties: individual maturity, respect for others, a team-centered approach, maintenance of confidential information, and awareness and sensitivity to cultural and other differences in others.
  • Exhibits effective communication skills, including proper use of agency communication systems.
  • Participate in appropriate professional development programs to attain and maintain competency.
  • Effectively manages financial and physical resources to achieve the mission of RE.
  • Report incidents of abuse or potential abuse involving clients to the appropriate authorities and RE.
  • Essential duties and Responsibilities:
  • Conduct thorough physical exams and health histories and evaluate the clients medical and psychosocial needs.
  • Analyze patient data and formulate appropriate diagnoses.
  • Prescribe medications and treatments within the scope of practice and as allowed by state regulations and collaborative practice agreements.
  • Educate clients and families, if applicable, about health management, preventative care, and lifestyle modifications.
  • Provide psychoeducation to the clients and families, if applicable, regarding medications and treatment plans.
  • Provides guidance on lifestyle modifications, nutrition, and self-care.
  • Develop and implement client-centered care plans based on diagnosis and client needs by setting measurable goals for client health improvement.
  • Conduct follow-up visits to ensure continuity of care
  • Collaborate with internal and external interdisciplinary teams to provide holistic care.
  • Participate in professional growth and development activities, including conferences or continuing education activities.
  • Interpret diagnostic or laboratory tests and communicate results with clients and families, if applicable.
  • Consult with other licensed professionals when encountering unusual or complex cases.
  • Refer patients requiring more specialized or complex treatment to appropriate providers.
  • Promotes a positive view of RE to the public and outside agencies.
  • Maintains current documentation, care plans, and other written documentation and correspondence according to agency policy and within appropriate time frames.
  • Maintains current licensure and stays within ethical guidelines provided by the licensure board.
  • Participates in supervision and consultation regularly.
  • Advocates for health education and wellness initiatives in the community.
  • Has adequate knowledge of healthcare regulations and standards.
  • Stay informed about public health issues and trends affecting the community.
  • Ability to be flexible and adapt to schedule and work environment changes.
  • Provides consistent, quality treatment to all clients.
  • Ensures all necessary records are maintained and kept confidential.
  • Performs other duties as assigned.
  • Working conditions/environment:
  • Shift is generally Monday Friday, but hours will vary based on the client's needs.
  • Holidays, weekends, and extra hours may be required.
  • Intense, unpredictable population with the possibility of verbal and physical aggression.
  • Fast-paced environment with the need for quick decisions to deal with any crisis that may arise.
  • Traveling weekly to meet with clients and families at different locations (Ashland, Rush, and Grayson offices).
  • Use of HIPPA-approved electronic programs to facilitate telehealth services.
  • Maintains a positive, professional attitude, contributing to a supportive work environment.
  • minimum job requirements:Education:Masters degree in nursing from an accredited program.Experience:Minimum of two years experience as a licensed Family Nurse Practitioner.Specific Skills andrequirements:Must be at least 21 years of age.Strong leadership and organizational skills.Excellent communication and conflict-resolution skills.Technical requirements include proficiency in Microsoft Word, Excel, PowerPoint, and other applications used by the organization or regulatory agencies.Valid Drivers License and insurability.Ability to understand and relate to the needs of clients from diverse backgrounds.Ability to read, write, and converse in English.Successful completion of a pre-employment drug screen.Successful completion of a background screening.Successful completion of a TB skin test or proof of a negative chest x-ray or other documentation.Specialized Licenses or trainings:Registered Nurse License.Certification as a Family Nurse Practitioner.Maintains 40 hours of Annual Training through Relias or other appropriate sources.Maintains at least minimum training for licensure.Attends all assigned training specific to job assignment.Physical Requirements:The physical requirements described here are representative of those that an employee must meet to successfully perform the essential functions of this job.
