We are seeking a dedicated and compassionate Resident Care Coordinator to join our team!
This position requires a Licensed Vocation Nurse (LVN) who is passionate about providing excellent care and support to members and their families. Some of the key responsibilities of this role include:
- overseeing and managing the nursing schedules to ensure adequate coverage
- collaborate with care teams to maintain high-quality services and address resident needs
- monitor compliance with care plans and regulatory standards
- assist with training and onboarding care staff as needed

- Review 24-hour log
- Receipt and record review for new move-in
- EMR Dashboard review and update
- Census review and update
- Review of incidents and regulatory required reporting
- Pharmacy Order updates in eMAR
- Document uploading
- Complete resident assessments and service plans
- Attend and participate in meetings
- Able to work weekends and holidays
- Able to work in a fast-paced environment
- Excellent work ethic
- Excellent communication skills
- Highly organized
