Details Client NameMonadnock Community Hospital - AA Job TypePermanent OfferingAllied ProfessionTherapy SpecialtyPT Job ID29908246 Job TitlePerm - PT - Rehab (Days) Peterborough, NH Shift Details ShiftDay - 12x3 - 07AM Scheduled Hours36 Job Order Details Start Date01/06/2025 Job Description Job Title: Physical Therapist (PT) Job Specialty: Rehabilitation Job Duration: [Please specify] Shift: Days, no weekends or holidays Guaranteed Hours: 37.5 hours/week (salaried position) Experience: 1-2 years preferred License: Must be licensed by the State of NH Certifications: BLS Certification required Must-Have: Bachelor's Degree with completion of an accredited program in PT Job Description: - Conduct patient/resident therapy evaluations and treatments according to established policies and procedures, catering to a range of age groups from pediatric to geriatric.- Evaluate patient/resident status and needs, and plan treatment while establishing realistic and measurable short-term and long-term goals with the referring provider, patient/resident, family/caregivers, and other team members.- Assess the effectiveness of therapy treatments and plans according to department policies and procedures.- Provide education to patients/residents, family/caregivers, and other team members to facilitate the attainment of established goals.- Deliver care/treatment suitable for the patient's/resident's age, physical ability, and intellectual development.- Complete accurate, legible, timely, and effective documentation according to department policies and procedures.- Communicate clearly and concisely with team members, referring providers, insurance/case managers regarding patient/resident status, progress, needs, and discharge planning.- Delegate appropriate patient/resident treatments to therapy assistant staff and provide supervision and direction to therapy assistants.- Complete reevaluations in cooperation with therapy assistants as indicated and according to department policy and procedure.- Ensure daily billing accuracy.- Schedule patients efficiently and in cooperation with the receptionist.- Engage in department and hospital-wide educational opportunities for continuing professional development/education.- Offer one department in-service session if attending a continuing education course reimbursed by the organization.- Participate in the Rehab Department student program, serving as a clinical instructor, mentor, and role model as appropriate.- Contribute to one Rehab Department or hospital project, process improvement initiative, or committee.- Adhere to all safety policies and procedures, reporting incidents, falls, adverse drug reactions, and equipment issues according to hospital policies.- Participate in the development of and adhere to the department's mission, values, and ground rules.- Embrace cultural changes and implementations within the organization.
