The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
Works closely with the Provider team to facilitate admission or transfer of patients from Strong Memorial Hospital Emergency Room to UR Medicine affiliate hospitals. Interviews and consents patients for transfer, answers patient and family questions regarding transfer to affiliate locations and completes appropriate forms for transfer. Interfaces with care team members including physicians, nurses, social work, and support staff to facilitate communication. Creates transfer center intake for service requests to be triaged and managed by the appropriate area leaders.

Interviews and consents patients for transfer, answers patient and family questions regarding transfer to affiliate locations and completes appropriate forms for transfer. Interfaces with care team members including physicians, nurses, social work, and support staff to facilitate communication. Creates transfer center intake for service requests to be triaged and managed by the appropriate area leaders. JOB DUTIES AND RESPONSIBILITIES: - Facilitates coordination between Strong Memorial Hospital and affiliate locations to arrange patient transfers to affiliate locations, consents patient for transfer and completes all required paperwork for transfer, under the direction of area leadership. Arranges transportation for transfer and liaisons with nursing to facilitate RN handoff. - Reviews patient for transfer based on geographic proximity to affiliate hospitals and necessary service, utilizing service availability lists and in discussion with the Strong Memorial Hospital provider team for the applicable department. - Documents in an accurate, timely manner in the EHR (electronic health record) transfer intake. Updates patient encounters for affiliate locations ensuring accurate insurance information, demographic information, service and accommodation information. - Answers incoming calls for service requests, enters accurate data in appropriate system (EMR) for RN follow up. - Collaborates with health care team to facilitate communication regarding transfer and updates department staff regarding transportation. - Completes daily calls to affiliate and outside hospitals relating to pending and accepted transfers awaiting bed assignments. Alerts appropriate leadership of any change in patient status communicated by the referring facility that may require clinical triage and follow up. Communicates bed assignment to referring facility, when identified by receiving facility, under direction of area leadership. - Perform other duties assigned by the manager such as statistical reports, correspondence, and other records maintenance, as needed. Other duties as assigned QUALIFICATIONS: - Bachelor's Degree in Health Care, Education, Psychology, or related discipline required - Master's Degree in related discipline - 3 years of related experience (advanced customer service in fast paced environment, registration, data entry, health care) required - Or equivalent combination of education and experience required - Ability to function in a fast-paced environment required - Knowledge of hospital policy and procedures as they related to transfers between affiliate locations required - Knowledge of and proficient ability to use the Electronic Health Record (EHR) required - Excellent written and verbal communication skills, as well as excellent customer service skills preferred - Ability to work independently as well as highly collaboratively in a team environment preferred - Strong computer skills and ability to type minimum of 45 words per minute preferred - Knowledge of medical terminology preferred - Professional, compassionate demeanor and excellent people skills preferred The University of Rochester is committed to fostering, cultivating, and preserving a culture of equity, diversity, and inclusion to advance the University's mission to Learn, Discover, Heal, Create - and Make the World Ever Better.
