**Job Title:** Registered Nurse I**Job Specialty:** Home Health**Job Duration:** 13 weeks**Shift:** Day 5x8-Hour (08:00 - 17:00)**Guaranteed Hours:** None**Experience:** Minimum 1 full year in Adult Home Health experience or Hospice experience**License:** Must have license in hand**Certifications:** CPR, BLS**Must-Have:**- Proficiency in HomeCare HomeBase used within the last 6 months- Proficiency in OASIS- Reliable transportation, driver's license, and auto insurance- Comfortable traveling up to 75 miles**Job Description:**- Provide, coordinate, and direct home nursing care based on physician's orders.- Responsible for supervising and evaluating Home Health Aides and Licensed Practical/Vocational Nurses.- Conduct home visits and attend periodic office meetings.- Manage an average of 5-8 patients per day with point values assigned to various activities.- Perform care tasks including Start of Care, Wound Care, and Follow-Up Visits.- Complete all documentation within 24 hours of patient appointment.- Maintain high productivity levels, equating to approximately 40 hours per week.- Participate in floating to nearby clinics when necessary to meet financial needs due to low patient census.- Dress code required: scrubs.- No weekend rotation or on-call requirements.- Cover assigned territory with potential to travel up to 200 miles per week.- Proficient use of HomeCare HomeBase EMR system.- Complete a retraining course if required based on recent experience with necessary systems.
