The Adult case management unit serves at-risk consumers with mental health diagnoses that have a variety of needs, such improving social wellbeing, behavioral and physical health, or could benefit from increased recovery skills and improved quality of life. Case managers in this unit assist consumers with increasing or maintaining mental health stability at the lowest level of appropriate care. Often clients we serve are entering the community out of psychiatric facilities, require ongoing support to maintain stability while living independently, or are deemed gravely disabled by the court and placed in facilities on LPS conservatorships. The Mental Health Counselor in this unit provides services to a caseload of up to 20 clients. Services are client centered and can range from intensive wraparound interventions to long term maintenance, and are provided in the office, the community, and in contracted facilities. These include targeted case management services, rehabilitative skill development, therapy, treatment coordination, collaboration with treatment team or other providers, advocacy and linkage to community resources as identified by client need, and collaboration with the other members of the clients support network. 1. Case Management Duties: Assess clients' needs and level of care, provide referrals and resources, collateral involvement with families and other professionals and with the treatment team. Assist clients in living in the least restrictive level of care possible. Individual and collateral support given through telephone, office and field visits. Coordination of services and assistance with accessing housing and financial resources, psychiatric and medical care, vocational and educational resources, etc. as needed by participants. Rehabilitative interventions aimed at building pro-social life skills, social skills, leisure skills etc. Advocate for consumers to obtain needed services provided by community resources and coordinate treatment planning with hospitals, board and care homes, IMD's, etc. 2. Documentation: Includes various assessments, progress notes, discharge summaries etc. in the electronic health record, as well as preparing packets required for accessing placement and other resources, releases of information, quarterly reports, and other needed documents for individual clients. All documentation to be legible, timely, and expressed concisely and clearly. 3. Meeting and court attendance: In collaboration with LPS conservator and LPS office assistant, plan for yearly LPS renewal hearings and provide support to client throughout this process. Attend Inpatient Treatment team meetings as well as unit meetings, educational workshops and agency committees when requested. This position may be filled as a Mental Health Counselor - Licensed or Mental Health Counselor - Register, depending on qualifications. The list established by this recruitment may be used to fill future full-time, part-time, limited term, and extra help vacancies up to one year. There are currently 2 (two) regular full-time vacancies. MOU-agreed salary increases and Winter Time Off (WTO): 3.5% COLA Increase effective 7/1/2025 3.5% COLA Increase effective 7/1/2026 32 hours of Winter Time Off Position Requirements Experience and Education: Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying. A typical way to obtain the knowledge, skills, and abilities would be: Mental Health Counselor - Register ( $ 46.99 - $56.02 / Hour) Experience: Two years experience in the provision of mental health counseling services. Education: Possession of a Master's degree from an accredited college or university in social work, Marriage and Family Counseling, clinical psychology, counseling, or a closely related field and registration by the State of California Board of Behavioral Science Examiners. License or Certificate:* Incumbent in this position is expected to operate automotive vehicles in the performance of assigned duties. Due to assignments and hours worked, public transportation may not be an efficient method for traveling to required locations. Individuals appointed will be required to maintain a valid California Driver's License while employed. *Any license, certification, or registration required for this position shall be maintained (i.e. active and in good standing) at all times during employment with Napa County. For continued employment with Napa County, you must maintain such license, certification, or registration to meet the minimum qualifications of this position. Loss of the required State licensure may result in termination or reassignment. Mental Health Counselor - Licensed ( $ 48.66 - $ 58.13 /Hour) Experience: Two years supervised experience in a mental health setting providing direct services to mentally or emotionally disabled persons, including dual diagnosis and addiction concerns for those assigned to the substance abuse setting. AND Education: Possession of a Master's degree from an accredited college or university in Marriage and Family Therapy, Social Work, clinical or counseling psychology, or a closely related field. Required License : Current license by the State of California Board of Behavioral Science Examiners as either a Marriage and Family Therapist (MFT), Licensed Clinical Social Worker (LCSW), or **Licensed Professional Clinical Counselor (LPCC); OR Current license by the State of California as a Licensed Psychologist. Additional License or Certificate:* Incumbent in this position is expected to operate automotive vehicles in the performance of assigned duties. Individuals appointed will be required to maintain a valid California Driver's License while employed. **Licensed Professional Clinical Counselor (LPCC) must include the additional training and education to treat couples or families, as required under BPC section 4999.20(a)(3)(A).*** *Any license, certification, or registration required for this position shall be maintained (i.e. active and in good standing) at all times during employment with Napa County.
