CarePlus TLC is seeking, energic, detail-oriented HHA/CNAs who are committed to creating a positive experience in our patient's lives. You will assist the Physician in providing care to patients by creating a friendly, caring environment. The successful candidate is self-motivated, compassionate and able to communicate effectively with patients and their families.

Client is located in Conklin. 2- 4 hours shifts on Tuesdays and Thursdays from 11a-3pm for a total of 8 hours per week. Pay is $17/hr.

• Provide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, etc.
• Assit with housekeeping duties
• Understands First Aid and Emergency Care protocols
• Complete electronic documentation while on shift
• Monitor patient's condition and report changes/concerns to appropriate medical professionals and family members
