Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. We strive to hire excellent people at Concordia - and keep them as well!POSITION SUMMARYThe Hospice Aide provides personal care and assists in activities of daily living and related services to hospice patients. This employee functions under the direction, instruction and supervision of the Registered Nurse (RN) Case Manager. Care for the patient is given where the patient resides (i.e., patient's home, facility, hospital).We are looking for Aides in the following areas:
- Upper Butler County & New Castle area
- North Hills/Allison Park/Sewickley area
QUALIFICATIONS•Current Certified Nurse Aide (CNA) in the state of Pennsylvania preferred but not necessary•Minimum of two [2] years experience in patient care preferred•Ability to understand and carry out directions and instructions and maintain appropriate recordsRESPONSIBILITIES1. Provides personal care according to the established individualized plan of care as developed by the RN member of the hospice team such as (but not limited to): bathing, grooming, dressing, shaving, feeding; checking vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation2. Coordinates and communicates a weekly patient schedule with the RN Case Manager and Licensed Practical Nurse (LPN) to meet the optimal needs of the patient and family3. Completes all computer and/or paper documentation requirements within the timeframes specified4. Participates in patient and/or family teaching activities as directed by the RN hospice team member5. Together with the hospice team, provides care as established by the IDG to meet the physical, emotional and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death6.
