Responsibilities:As a Family Liaison, you'll provide essential support to parents and individuals within theCommunity Services Community Habilitation Program. Key duties include:
- Care & Support: Assist families in navigating and coordinating services, ensuring that needs are met in alignment with program goals.
- Collaboration: Work closely with families, service providers, and community partners to ensure seamless communication and access to available resources.
- Self-Directed & Co-Managed Services: Receive training and instructions from individuals or their designees, ensuring services are provided according to personalized guidelines and protocols.
- Advocacy: Help families understand and access services, offering guidance and support to ensure informed decision-making.
- Documentation: Maintain accurate and up-to-date records of services, communications, and progress, meeting program and regulatory standards.
- Empowerment: Support families in understanding their rights and options, promoting independence and informed decision-making within the program.
- Community Engagement: Help families connect with community resources and foster a network of support.
- Additional Support: Attend training, meetings, and workshops to stay informed on best practices and program developments.
Qualifications:
- Bachelor's degree in field of education, social work, psychology, rehabilitation, or
related human service field from an accredited program and 2 years of experienceserving individuals with developmental disabilities with a minimum of 2 additionalyears working as a case manager in the MH or DD field. OR:
- Associate's degree in Human Services and 2 years of experience serving individuals
