Job DescriptionTo abstract and analyze patient medical records and other sources of data for the purpose of Quality Assurance and Performance Improvement, and to provide support for the Quality Management team and entire Quality department in an effort to advance high-quality, equitable care provided to patients and workflow for all employees.

Unit Specific:

Performs primary data abstraction duties for Core Measures and registries and ensures high levels of abstraction accuracy for assigned work via validation activities for both Jacobi and North Central Bronx.

Demonstrates strong communication, documentation, organizing and planning skills to ensure strong leadership of multiple accounts concurrently.

Performs Ad hoc or special project data abstraction, entry and analysis in order to support the Quality Assurance Program.

Performs data analysis and provides visual aids, and graphical representations of data as well as recommendations and support to help clinical and other departments improve care.

Assists in Data Submission to CMS, Joint Commission, Department of Health, and various quality programs.

Pivots to the evolving responsibilities/needs of the Quality department.

Familiarity with medical records, billing/documentation practices, and standard healthcare quality concepts.

Reviews Patient Files for any quality of care concerns or documentation incongruencies, and follow up with clinical areas for appropriate remediation.

Supports the Director of Quality Management and the Chief Quality Officer in the implementation of various performance improvement/quality/risk/regulatory/patient safety initiatives, for the betterment of patient care and alignment with the system-wide priorities. This may include facilitating groups through evidence-based quality improvement activities using proven QI methodology and tools such as Fishbone Diagrams, Pareto Charts, Decision Matrices, Stakeholder Assessments, and Root Cause Analysis. Performing project management, and at a minimum conduct meetings, compile action trackers, and monitor completion of deliverables.

General:

Makes improvements in the operations of the department, provides recommendations and implements approved changes, including revising procedures or devising new forms or methods to improve efficiency of workflow.

Works collaboratively and supportively with the Risk, Regulatory, Patient Safety, and Infection Prevention departments.

Minimum Qualifications
  • Qualification for Central Office.
Qualification for Health Care Facility:1. a. A Baccalaureate Degree in Public Administration, Public Health or Health Care Specialization; and,b. Five* years or four# years of increasingly responsible experience in hospital administration, with particular emphasis on development and evaluation of health care delivery services; or,2. a. A Masters Degree in Public Administration, Public Health or Health Care Specialization; and,b. Four* years or three# years of increasingly responsible experience in hospital administration, with particular emphasis on development and evaluation of health care delivery services.Department Preferences
  • Licensed by NYS Office of the Professions in a health care profession (i.e RN, OT, PT, Radiology, Pharmacy). Experience in Quality, Performance Improvement, data analytics and/or hospital operations. Proficiency in Microsoft Office Suite including Excel, Visio and Access, Epic EHR, and electronic registries. Experience preferred in state and national quality registries including Get with the Guidelines Stroke/HF and Core Measures CPHQ, CPPS Certifications.
  • Nationally recognized quality/safety certification such as CPHQ and/or CPPS preferred.
  • Excellent interpersonal and communication skills.
