You will be explaining the Our Hospice program to referral patients, advising on appropriateness of admission based on the Our Hospice admission criteria, and completing their admission into our care. This position is often the first interaction that the patient and their family has with Our Hospice and hospice care in general. This is a great opportunity if you're looking for more of the one-on-one patient care and education.Hours: Friday, Saturday and Sunday 10 hours per day, OR Saturday and Sunday 12 hours per day (schedule to be determined between employee and manager)No on-call and cover one holiday per yearLocation: Will cover all service areas Bartholomew, Decatur and Jennings and surroundingPay: $37+ based on experienceMileage reimbursed at 0.52 cents per mileAn Admissions RN at Our Hospice Will:Make the initial contact on referrals as required, explaining hospice services and evaluating the patient's ability to meet Our Hospice's admission criteria.Promote the hospice benefit and provides education to all potential referring customers including but not limited to case managers, physicians and physician office staff, and community groups.Perform as admission nurse in the home, extended care facilities, and assisted living facilities.Complete admission and begins assessment of patient/family needs including physical, emotional, and bereavement status.Build collaborative relationships with Referral and Admission Team members, Interdisciplinary Teams, Referral Coordinators, and others to assure coordination and continuity of care and services.Review relevant medical records, assessing for documentation for support hospice medical eligibility and goals of care.Evaluate patient/family outcomes and satisfaction with care/services provided.An Admissions RN at Our Hospice Needs:Associate's Degree in Nursing Science (ASN) or Bachelor's Degree in Nursing Science (BSN)One year experience in health care preferredLicensed Registered Nurse in the State of IndianaValid Driver's LicenseReliable vehicle for home visitsOur Hospice employees are eligible for Public Service Loan Forgiveness (PSLF)#INDHSCI2We are an equal opportunity employer at Our Hospice of South Central Indiana (OHSCI).
