Your responsibilities will include:

• Conducting initial and ongoing comprehensive assessments of patients.
• Monitoring changes in patient status and determining necessary service modifications.
• Documenting treatment interventions and patient progress accurately.
• Educating patients, families, and caregivers to foster progress towards established goals.

Qualifications required:

• Current Arizona licensure or certification as a Physical Therapist.
• Minimum of one year of experience in Physical Therapy.
• Preferred experience in a home care setting.
• Valid Arizona Fingerprint Clearance card.
• CPR Certification (course demonstration required).
• No Covid vaccination required.
