Location: Brooklyn, NY

Work Setting: In-person. Half-day in clinic and half-day in participant's home.

Salary: $99,195.45 - $113,800.12

Schedule: Monday - Friday, 9:00 am - 5:00 pm ET

About the Role

We are seeking a dedicated and compassionate Occupational Therapist to join our interdisciplinary team. In this role, you'll design and deliver treatment plans aimed at restoring function, alleviating pain, and preventing disability for our participants, ensuring they can thrive both in our clinics and their homes.

Key Responsibilities

  • Conduct participant evaluations and collaborate with providers on treatment programs.
  • Complete required assessments, progress notes, and documentation per departmental policies.
  • Deliver therapy both at the PACE site and in participant homes or nursing facilities.
  • Develop and participate in care plans with interdisciplinary team members.
  • Ensure a clean and safe environment while adhering to safety protocols.
  • Provide clinical education to students and staff, including COTAs and volunteers.
  • Recommend equipment for discharges and participate in Performance Improvement Monitoring.

Qualifications

Education:

Bachelor's Degree in Occupational Therapy from an accredited university (or equivalent).

Experience:

  • Minimum of 1 year as an Occupational Therapist (experience with the geriatric population preferred).
  • Experience working with frail or elderly populations is a plus; training provided for candidates with less than one year of experience.
  • Knowledge of PACE regulations and Care Compass or similar EMRs preferred.

Licensure:

Active New York State Occupational Therapist license.

Additional Requirements:

  • Legal authorization to practice in NY.
  • Up-to-date immunizations and medical clearance for communicable diseases.
  • Bilingual candidates preferred (based on participant demographics and site location).
