1. Assists with basic care for patients in the areas of nutrition, elimination, comfort, mobility and safety in patient homes .. Provides more individualized care as delegated by the RN though reinforcement of education providing dressing changes, and by providing household services essential to health care at home (ex. dishes, laundry, simple meal prep).3. Maintains visit schedules in the patient's home as identified by the multidisciplinary health care team and the patients and families .. Implements the patient and family care plan under the indirect supervision of the RN.5. Works collaboratively with the multidisciplinary team members to effectively communicate and document accurate information ..
