Job Description :

What is an Assertive Community Treatment (ACT) team? An ACT Team is a multidisciplinary group that provides comprehensive, community-based psychiatric treatment, rehabilitation, and support to individuals with severe mental illnesses. ACT teams offer services directly in the community, including medication management, therapy, crisis intervention, and assistance with daily living activities. The goal is to reduce hospitalizations and improve the quality of life by helping individuals live independently and achieve their personal recovery goals. DUTIES AND RESPONSIBILITIES 1. Develop an Integrated Treatment Team Plan (ITTP) and facilitate implementation of the Individual Program Goals. Goals can include, but not limited to Community Living Skills, Daily Living Skills, Interpersonal Skills, Vocational/Educational Skills, Medication Management, Physical Health Care, Mental Health Services, Substance Abuse Services, Resource Acquisition, Psychiatric Emergency/Relapse. 2. Conduct a needs assessment in client's place of residence. 3. Arrange for and physically transport clients as needed to facilitate treatment plan. 4. Develop crisis plan; assess and respond to crisis situations as well as developing plans of actions with the client. 5. Monitor client progress in the services received and facilitate updating the individual service goals. 6. Coordinate all treatment activities and services with the Treatment Team while participating in the on-call rotation. 7. Advocate for the client with other service providers. 8. Develop discharge plan with the client. 9. Treat all individuals with dignity and respect while maintaining agency standards of confidentiality, professionalism, and objectivity. 10. Maintain all HIPAA and 42 CFR confidentiality requirements and regulations as outlined in CARF policies and procedures. 11. Comply with agency policies, procedures and regulatory standards. 12. All other duties as assigned. QUALIFICATIONS/SKILLS/KNOWLEDGE REQUIRED FOR POSITION 1. Bachelor's degree in social work, human services, sociology, psychology or education or related field. 2. Two years' demonstrable knowledge, familiarity, and/or experience in the provision of community-based rehabilitation services. 3. Demonstrated decision making skills and competencies to work with adults with severe and persistent mental illness. 4. Familiarity with psychotropic medications and their side effects. 5. Experience in problem-solving and using discretion in crisis mediation situations. 6. Valid vehicle operator license, proof of liability insurance and ability to drive for organization business. 7. Ability to work a flexible schedule, including evenings and/or weekends if necessary. 8. Travel may be required as this is a community-based position. 9. Supportive of the mission of Prairie Ridge and the patients we serve. 10. Proficiency in oral and written communication skills, as well as documentation requirements. 11. Technology skills allowing for proficiency in use of electronic health record. 12. While performing the duties of the job, the employee is regularly required to sit; use hands to finger, handle, or feel objects, tools, or controls; and talk or hear. The employee frequently is required to walk and stoop, kneel, crouch, or crawl. The employee is often required to stand, reach with hands and arms, and climb or balance. 13. The employee must infrequently lift and/or move up to 10 pounds and occasionally lift and/or more up to 35 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, and the ability to adjust focus.
