Summary Description: Greater Mental Health of New York's Care Management Program helps individuals access essential services, such as medical specialists, transitioning from hospital admissions, and securing social supports such as housing. The Care Manager is responsible for delivering comprehensive care management services to individuals qualifying for the Health Homes Care Management program in Westchester County. This includes assessing, overseeing, and coordinating various needs such as medical, behavioral health, substance abuse, and social services. The role involves direct client care in the community or via telephonic services, participation in meetings, and adherence to agency policies while supporting recovery and utilizing evidence-based practices. Key Responsibilities:
  • The care manager will manage a caseload of clients consistent with industry standards, meets regulatory expectations/ guidelines and reflects our client population's needs. The exact number of clients may vary based on factors such as case complexity and the availability of support resources. The care manager is expected to maintain a balanced caseload for high-quality care and effective case management.
  • Provide comprehensive care management and care coordination, through face-to-face and telephonic services.
  • Completes assessments and approves applications for services and entitlements
  • With client's input, creates individualized plans of care
  • Assists in discharge planning during a client's hospitalization and provides follow-up care as needed
  • Consults with team members and other specialists
  • Participate in meetings with community agencies and service providers.
  • Follow established agency policies and maintain professional standards.
  • Prepare required reports and documentation.
  • Provide after-hours emergency coverage and crisis intervention as needed.
  • Demonstrate flexibility with working hours.
  • Adhere to standards established by regulatory authorities.
  • Perform other duties as assigned.
Key Competencies:
  • Knowledge of behavioral diagnoses and symptomatology.
  • Familiarity with community resources.
  • Effective client engagement skills.
  • Understanding of chronic disease/behavioral health interactions.
  • Ability to work independently and efficiently.
  • Proficiency in using Electronic Health Record systems.
  • Practice in a recovery-oriented, person-centered methodology.
Requirements:
  • Bachelor's degree in a relevant field or equivalent.
  • E.g. social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field
  • Two years of relevant experience or equivalent. A master's degree in one of the listed education fields may be substituted for one year of Experience.
  • Basic computer knowledge.
  • Valid Driver's License and satisfactory driving record.
  • Maintain New York State Defensive Driving certification within 30 days of hire.
  • Must maintain pre-employment clearances by applicable regulatory clearances as required.
  • Legal eligibility to work in the United States.
