Key Responsibilities:

• Undertake comprehensive assessments of individuals with learning disabilities, identifying their needs and strengths.
• Develop and implement person-centered care plans that promote independence and well-being.
• Provide support and guidance to individuals and their families, addressing complex challenges and promoting positive outcomes.
• Collaborate with multidisciplinary teams, including healthcare professionals, housing providers, and other relevant agencies.
• Advocate for the rights and needs of individuals with learning disabilities, ensuring they have access to appropriate services and support.
• Maintain accurate and up-to-date case records, adhering to professional standards and legal requirements.

Essential Requirements:

• Social Work qualification (Degree or Diploma level)
• Registration with Social Work England
• Proven experience working with individuals with learning disabilities
• Strong assessment and care planning skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Commitment to promoting person-centered practice
• Flexibility to work outside of traditional office hours as required
