Key Responsibilities:
- Undertake comprehensive assessments of individuals with learning disabilities, identifying their needs and strengths.
- Develop and implement person-centered care plans that promote independence and well-being.
- Provide support and guidance to individuals and their families, addressing complex challenges and promoting positive outcomes.
- Collaborate with multidisciplinary teams, including healthcare professionals, housing providers, and other relevant agencies.
- Advocate for the rights and needs of individuals with learning disabilities, ensuring they have access to appropriate services and support.
- Maintain accurate and up-to-date case records, adhering to professional standards and legal requirements.
Essential Requirements:
- Social Work qualification (Degree or Diploma level)
- Registration with Social Work England
- Proven experience working with individuals with learning disabilities
- Strong assessment and care planning skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Commitment to promoting person-centered practice
- Flexibility to work outside of traditional office hours as required
