Adults Social Worker, Northwest LondonAccess and Wellbeing TeamPay rate £32 per hourVitalis Social Care are recruiting for an experienced and qualified Social Worker to join an Access and Wellbeing Team in the Northwest London region.Job Role and Responsibilities: - The Assessment and Wellbeing Teams provide a supportive service applying preventative, enabling, proactive and strength-based approach. The teams support with medium to a longer-term social work support and interventions. Along with safeguarding enquiries, reassessment and reviews.You will have experience of completing Care Act assessment, Court of Protection and Human Rights assessments and to carry out joint NHS assessments.
