Social Worker | Intergrated Hospital Team | Wokingham Borough Council

We are looking for a Social Worker with current Social Work experience to join a busy front door Social Work team. This team is a mixure Hospital Discharge, Phsical Disabilities and Reablement . The Social Worker will based in the Wokingham.

Duties

• Undertaking direct work with customers who have a learning disability, physical disability, long term conditions and the elderly frail.
• The main aim of the service is to enable individuals to live safely in the community, maximising individual’s abilities to care for themselves and supporting carers to continue in their caring role as long as they are able to.
• Working in accordance with key legislative framework, the service will support individuals to access support services in line with their eligible needs, with an emphasis on health, safety and wellbeing for vulnerable individuals

To be considered, you must have a Social Work qualification (Diploma in Social Work, Social Work Degree or equivalent) as well as at least 3 years post qualified experience working as a Qualified Social Worker within a similar team.

£500 bonus if you refer a suitable person for this job
