About the Team

  • This team supports Lambeth residents who are admitted to Kings College Hospital (KCH), Guys and St Thomas’ Hospital (GSTT) and hospitals out of the borough. 
  • The primary feature of work undertaken by staff in the Hospital team is to deliver strength based preventative support, where required assess and establish supports and safeguard patients. The team undertake reviews of existing packages of care and consideration of placements. 
  • Assessors carry out a variety of responsibilities, including signposting, providing information and advice, supporting Duty, referring people to reablement and ensuring safe and timely decisions are made to facilitate discharges. 
  • The team work closely alongside our MDT colleagues and voluntary sector organisations.  
  • The Hospital is a fast-paced team with a committed and enthused workforce who strive to achieve positive outcomes for our residents.  
  • You will work closely with other teams in the borough and engage wider agencies to support the person. 
  • You’ll need to adapt to a demanding and changing environment and have a commitment to partnership and collaborative working.

About the Role:

We are looking for an experienced and compassionate Assessor to join our dynamic team. As part of this integrated service, you will play a key role in supporting individuals in the community, ensuring their needs are met through person-centred care and collaborative teamwork.

Key Responsibilities:

  • Care Act Assessments & Support Planning: Carry out thorough assessments and develop personalised care plans to support Lambeth residents.
  • Home Visits & Safeguarding: Conduct urgent face-to-face visits and address safeguarding concerns.
  • Carer Support: Assess the needs of carers and ensure access to the right support and services.
  • Mental Capacity Assessments: Complete mental capacity assessments within the legal framework, ensuring that all individuals are supported appropriately. 
  • Duty Coverage & Multidisciplinary Collaboration: Work alongside health professionals to screen new referrals and participate in regular multi-disciplinary team meetings. 
  • Rehabilitation & Reablement Services: Identify and support cases that require the Rehab & Reablement pathway, helping residents regain their independence
