Job Overview:We are seeking a Band 6 Occupational Therapist to join the Knowsley Community Therapy team at Merseycare NHS Trust. In this role, you will deliver high-quality therapy services, supporting patients to regain independence and achieve their goals in a community setting.

Key Responsibilities:

• Conduct comprehensive assessments and develop tailored intervention plans.
• Support patients in achieving rehabilitation goals within their homes and local communities.
• Work collaboratively with multidisciplinary teams to deliver holistic, patient-centered care.
• Provide education and advice to patients, families, and carers to facilitate recovery.
• Maintain accurate documentation and comply with trust policies and professional standards.

Key Requirements:

• HCPC registration as an Occupational Therapist.
• Experience in community-based occupational therapy services.
• Strong communication, organizational, and interpersonal skills.
• Ability to manage a varied caseload and work independently.
• A valid UK driving license and access to a vehicle are desirable for community visits.
