Care Navigator / Health and Wellbeing Support Workerwith experience in supporting vulnerable adults and excellent verbal and written communication skills, is required to join a well-established Charity.
You will be working with people in prison, and people on probation in the community.
SALARY: £22,004 per annum + Excellent Benefits (see below)
LOCATION: Durham, North East England - covering the North East Prisons
JOB TYPE: Full-Time, Permanent
WORKING HOURS: 35 hours per week
KEY REQUIREMENT:Applicants must be 18+ due to prison based working and hold a Full UK Driving Licence with access to own transport.
The post will be subject to a DBS Enhanced with Adults barred check and Home Office Prison Clearance.
JOB OVERVIEW
We have a fantastic new job opportunity for a Care Navigator / Health and Wellbeing Support Worker with experience in supporting vulnerable adults and excellent verbal and written communication skills.
As the Care Navigator / Health and Wellbeing Support Worker you will work with other health professionals, care navigators and apprentice health and wellbeing coaches to ensure holistic and meaningful support is provided to those with health vulnerabilities on their release from prison, ensuring continuity of care and fruition of release plans.
Working as the Care Navigator / Health and Wellbeing Support Worker you will use a coaching framework to manage wellbeing in the pre and post release period, to prevent a return to ill health as well as a reduction in reoffending.
By preventing this decline in health, through robust reconnection with health services in the community, will reduce health inequalities for those leaving the criminal justice system and provide support to allow a seamless, and effective transition to independent community living.
