Pathways Social Worker
- Annual Salary: £32,654 - £42,708 plus a 15% market supplement paid monthly

Join our dedicated team as a Pathways Social Worker, where you will support young people aged 16-25 transitioning from care to independence. This role is crucial in designing and implementing pathway plans that promote young people’s move into independence, ensuring a smooth transition into adulthood.
Day-to-day of the role:
- Undertake a comprehensive Social Work service to Children and Families within the leaving care service.
- Formulate and design appropriate pathway plans to meet identified needs and arrange the implementation of such plans with an emphasis on promoting independence.
- Provide ongoing support to individuals and families as the key worker, including home visits, meetings, direct work/observations, and completion of reports and assessments related to the child’s pathway plan.
- Ensure individual professional standards are maintained in line with agreed levels, including full compliance with policy and procedural guidelines.
- Regularly feedback to the Team Manager on any issues related to the provision of services or policies.
Required Skills & Qualifications:
- A recognised Social Work qualification, e.g., CQSW, CSS, Dip SW.
- Registration with Social Worker England (Social Work England).
- Must have a full driving license and use of a car with business insurance.
- Demonstrable experience with service users, particularly looked after children and care leavers.
- Strong interpersonal, organisational, and partnership working skills.
- Ability to undertake assessments of service users and carers.
- Knowledge of the legislation relevant to looked after children and care leavers.
