Role Summary:Minerva Health Centre is seeking a dedicated Community Dietitian to deliver specialized dietetic assessment, advice, and care planning for community-based patients, particularly vulnerable elderly individuals with nutritional needs. This role involves close collaboration with multidisciplinary teams (MDT) to provide comprehensive, safe, and effective care, including specific support for patients requiring Home Enteral Feeding (HEF).

Key Responsibilities:

• Conduct dietetic assessments and develop individualized care plans to support community patients’ nutritional needs.
• Collaborate with other healthcare professionals to ensure a multidisciplinary approach to elderly care.
• Provide specialized oversight and support for HEF patients, ensuring safe and effective nutritional management.
• No remote work; role requires in-person patient interaction and team collaboration.

Essential Skills and Experience:

• Experience in dietetic assessment and care planning within a community or elderly care setting.
• Ability to work effectively within a multidisciplinary team to support vulnerable patients.
• Strong communication and interpersonal skills, with a focus on patient-centered care.
