Community Dietitian

Up to 4 days a week these will be set at 7.5 hours per day but which days can be negotiated

• Providing dietetic assessment, advice and care planning to community patients
• Working alongside other health professionals to provide a MDT approach to the care of vulnerable elderly patients with nutritional needs.
• Work with HEF patients to ensure their care is safe and effective Remote work not an option
