EHCP and Transitions Officer

- Hours: 8.00am – 4.00pm, 37.5 hours per week
- Accountable to: Headteacher
We are supporting a school in seeking an EHCP and Transitions Officer to join their dedicated team. The school is a Special School for students with Profound and Multiple Learning Disabilities, severe sensory impairment, and complex medical needs.
Day-to-day of the role:
- Coordinate and administer the EHCP and review processes, ensuring compliance with statutory timelines.
- Organise and invite stakeholders to review meetings, facilitating and chairing as needed.
- Take minutes for Annual Review meetings and other required meetings.
- Liaise with Local Authorities, Social Care, and Health Professionals, as well as the school’s multidisciplinary team.
- Coordinate transitions into the school for new students, including tours, assessments, and induction processes.
- Update school prospectus and materials for prospective parents.
- Collaborate with teachers and families to co-produce transition plans for students.
- Provide transition support for families exiting the setting.
- Develop external links with services such as work experience placements and other settings.
- Host and lead training for families on transition-related topics.
- Track destination data and provide family support and signposting.
- Seek feedback on transitions from stakeholders.
- Act as Deputy Designated Safeguarding Lead.
- Assist with general school office duties and administrative support for the headteacher.
- Work collaboratively with finance and HR to ensure smooth administrative functions.
Required Skills & Qualifications:
- Knowledge of SEND and EHCP procedures.
- Excellent communication skills, both written and verbal.
- Understanding of safeguarding children and vulnerable adults.
- Familiarity with Data Protection and confidentiality principles.
- Understanding of SEND code of practice and disability legislation.
- Comprehension of the school’s ethos, values, and the transitions process.
- Ability to establish and maintain positive relationships with staff and external support.
