Newly Qualified Social Worker - PPR Social Care : Job Details

Pertemps Social Care are recruiting for a newly qualified social worker, for an agency role in Haringey.As part of an Adults Locality Assessment team, you'll play a crucial role in providing tailored support and services to help individuals lead fulfilling lives.You will;

• Conduct comprehensive assessments of adults' needs and circumstances.
• Develop and implement person-centred care plans to support individuals in achieving their goals.
• Work collaboratively with health professionals, care providers, and other agencies to ensure integrated care.
• Provide guidance, support, and advocacy to adults and their families.
• Monitor and review the effectiveness of care plans, making adjustments as necessary.
• Maintain accurate records and prepare reports in line with statutory requirements.
