An exciting role has recently become available for a qualified Social Worker. The Barnsley Metropolitan Borough Council is looking for a new member to join the Hospital Social Work team in Barnsley. The pay rate is £35 per hour. This job is hybrid working but they need someone to be able to respond to urgent requests to visit anywhere in the borough. They work with a combination of home working, office based and hospital based.
What will your responsibilities be?You must have strong social work values that leads your practice with people in helping them achieve the best outcomes.We need someone who can be local to the area, as we are a reactive team, so you will need to be able to work agile, but be available at short notice to go out and complete the assessments etc. You must apply knowledge of the Discharge to Assess Process to support individuals transitioning from hospital. You should be able to collaborate with health colleagues across acute and intermediate care settings.You will also complete Care Act and Mental Capacity Assessments, and lead on best interest decision-making.Conduct safeguarding investigations and contribute to complex discharge planning using a strengths-based approach.

Qualifications and Experiences:
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have at least 12 months post-qualified Social Worker experience.
