Qualified Social Worker - Hospital Discharge - Tripod Social Care : Job Details

We are looking for a Qualified Social Workers to join a Hospital Discharge Team in Central London

The roles will pay up to £32ph.

• To deliver excellent social work practice, assessing complex need, planning and working purposefully with families, support networks and other professionals to manage risk, safeguard children and promote positive outcomes.
• To provide a social work service which is both relationship based and which responds in a timely manner within statutory guidelines.
• To work collaboratively with the team, supporting the development of best practice and confident practitioners through coaching and mentoring of students, newly qualified social workers and less experienced practitioners.
• This role is predominantly place-based in the hospital discharge team and visiting patients on the ward for complex discharge assessments and dedicated discharge planning, including mental capacity assessments and discharge planning meetings with medical colleagues, therapists and/or social care professionals and families.

To be considered, you must have a Social Work qualification (Diploma in Social Work, Social Work Degree or equivalent) as well as at least 1 year post qualified experience working as a Qualified Social Worker within a similar team.
