Job Title: Registered Nurse - Immediate Care Bed Base Team

Location: Ings Grove, Mirfield - WF14 8DP

Hours: 37.5 hours per week for a 3-month period (The service is open 7 days a week, 08:00 - 20:00)

Pay: £17.21 - £29.61 per hour

Job Description:

We are seeking a dedicated and compassionate Registered Nurse to join our client's Immediate Care Bed Base Team. This role involves providing short-term care, support, and rehabilitation to patients who have experienced a sudden onset of ill health. Their service bridges the gap between hospital and home, aiding patients in their recovery to ensure they can manage independently at home, thus preventing unnecessary hospital admissions or premature entry into long-term nursing or residential care.

Job Summary:

• Responsible for the assessment, planning, implementation, and evaluation of evidence-based individualised programs of care.
• Coordinate, manage, and organise the nursing caseload.
• This position may include internal rotation into other Intermediate Care Bed Bases.
• The post will include working weekends and late shifts.

Key Responsibilities:

• Deliver high-quality nursing care to patients in the intermediate care setting.
• Support patients in their recovery process, promoting independence and well-being.
• Collaborate with a multidisciplinary team to develop and implement individualised care plans.
• Monitor and assess patients' conditions, adjusting care plans as necessary.
• Provide education and support to patients and their families regarding care and recovery.

Requirements:

• Registered Nurse with a valid NMC pin.
• Experience in intermediate care or a similar setting is preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively within a multidisciplinary team.
• Commitment to providing compassionate and patient-centered care.
