Service Care Solutions are looking for SEND Key Workers to within the Cheshire East Council on a 3-month contract.Pay: £280per day umb ltdLocation: MacclesfieldJob Role/Responsibilities:To undertake statutory assessments and the development and maintenance of Education, Health, and Care Plans (EHCP) across the Local Authority for Children with Special Education Needs and Disabilities (SEND). Maintaining a caseload of children with EHCPs and attend annual reviews and make changes to plans as required.
- Co-ordinate a person-centred, outcome-focused multi-agency approach to assessment, planning, and support, ensuring young people and parents are treated as equal partners in the EHCP process. Act as the key worker for children and young people requiring SEND needs assessment and/or EHCP.
- Serve as the Named Officer and primary contact for named settings, schools, colleges, and specialist institutions attended by SEND children and young people.
- Attend multi-agency co-production meetings. Analyse and evaluate educational, medical, social care, and psychological advice, outcome-focused service delivery recommendations.
- Meet with children, young people, parents, and carers to resolve SEND support issues, including mediation, and direct families to advocacy services as needed.
- Submit evidence to Moderation Panels with sufficient details for informed decision-making.
- Support the wider SEND team in promoting an inclusive approach, advising families on SEND support provision.
- Represent the Local Authority in mediations and appeals.
- Monitor commissioned provision to track progress and outcomes, taking action to ensure individual goals are achieved.
- Liaise with non-educational agencies to ensure appropriate non-educational services are provided. Advise schools on meeting SEND needs and optimizing SEND funding use.
- Allocate, monitor, and review resources to support EHCP implementation.
Qualifications:
- Graduate level qualification, or equivalent, in a relevant field (e.g. Education/Social Work/ health related /Educational Psychology or similar NVQ Level 4 - Advice & Guidance/ NVQ Level 4 in learning, development and support services for children, young people and those who care for them (LDSS)/ Post / DIP Career Guidance/Diploma/degree Youth and Community/PGCE/Social Work qualification/equivalent etc.)
Knowledge/Experience required:
- Extensive experience across early years, schools, colleges, Local Authorities, health, or independent settings young people with SEND.
- Proven success in collaborative and partnership working with school staff, partner agencies, and families.
- Skilled in effective multi-agency working, including Statutory Assessments.
- Experienced in reviewing, monitoring, and evaluating performance against targets.
- Proficient in presenting evaluative reports to key professional audiences.
- Strong knowledge of current theory and practice in SEND related to areas of responsibility.
- Understanding of relevant legal frameworks.
- Knowledgeable in person-centred processes, outcome measures, and child-focused practices.
- Familiar with statutory frameworks for SEND and approaches to assessment, planning, and reviewing provision.
