Job Title: Senior Occupational Therapist (Band 6) - November start dates available!

Location: Kirklees, UK

Hours: 37.5 hours per week

Accountable To: Team Leader for Unplanned Care and Clinical Nurse Lead

Job Purpose:

We are seeking a skilled and compassionate Occupational Therapist to join our client's team. This role involves providing holistic assessments for adults with complex physical and medical needs, including those at risk of falls, and with neurological, musculoskeletal, and frail conditions. The goal is to work with key stakeholders and community services to avoid hospital admissions and assist in the timely discharge of those receiving care.

Key Responsibilities:

  • Conduct holistic assessments for adults with complex needs to avoid hospital admissions and support timely discharges.
  • Manage clients over the age of 18 requiring support discharge from acute settings, including those with complex health and social needs.
  • Provide advanced Occupational Therapy assessments and treatments for patients with complex physical and psychological symptoms.
  • Educate and support other healthcare professionals, carers, and students in managing clients with physical disabilities and cognitive impairments.
  • Supervise and mentor rehabilitation and healthcare assistants and Occupational Therapy students.
  • Work autonomously within a multi-disciplinary team in community bed facilities or other appropriate settings.
  • Participate in audits and research, disseminating findings and recommending changes to practice.
  • Collaborate closely with carers and health and social care agencies to ensure comprehensive care.

Principal Responsibilities:

  • Deliver comprehensive holistic therapy assessments, setting short and long-term goals to maximize patient independence and ensure safe discharge.
  • Provide specialist advice and support across the organization regarding the diagnosis and management of clients with multiple and complex disabilities.
  • Facilitate education and skill-sharing among rehabilitation assistants and healthcare assistants.
  • Liaise with other professionals to ensure the best interest of clients.
  • Monitor and evaluate patients’ conditions, adjusting care programs as needed.
  • Maintain accurate and comprehensive patient records.
  • Demonstrate effective communication skills, particularly in complex cases, to establish therapeutic relationships and motivate patients.
  • Help patients cope with diagnosis/prognosis and facilitate lifestyle adjustments.

Clinical Responsibilities:

  • Demonstrate knowledge and experience in discharge planning and interdisciplinary working.
  • Manage clients with challenging behaviours and apply appropriate management strategies.
  • Undertake comprehensive assessments of clients with complex presentations.
  • Manage clinical risk within your caseload.
  • Interpret and analyse clinical and non-clinical facts to form accurate diagnoses and recommend safe discharge plans.
  • Promote patient responsibility for their own rehabilitation, utilizing a maximizing independence approach.

  • A valid driving license and access to a car.
