Location: Cardiff, CF5 and CF64Contract length: Until January 2025 and a chance of extension from therePay Rate: £16.00 per hour umbrellaWorking Hours: 37 hours per week, Monday to Friday

We have one role to work in a Day Centre for Adults with Learning disabilities. And another wot work at Llandough hospital, This role will consist mainly of completing reviews / assessments under the Social Services and Well-being Act.

Duties include:• To arrange and monitor appropriate services in line with individual plans.• To provide advice and practical help.• To liaise with voluntary and statutory agencies.• To assist the case managers in the team in the carrying out of specific tasks• To participate in team meetings and duty rotas• To undertake these duties in line with County Policies and Procedures.• To undertake training and self-development and to contribute to this process in others.
