Conwy Council is seeking a dedicated and empathetic Social Worker to join our Older People & Hospital Team. Key Responsibilities:

• Conduct assessments, develop care plans, and provide interventions for older adults transitioning from hospital care to community living.
• Work with multi-disciplinary teams including healthcare professionals, discharge coordinators, and community services to ensure smooth and safe discharges.
• Provide ongoing support to older adults and their families, promoting independence and well-being.
• Advocate for older individuals, ensuring their needs are met through appropriate care and services.

Requirements:

• A recognised Social Work qualification.
• Registration with Social Care Wales.
