Job OverviewGreater Manchester Mental Health are currently looking for an experienced Social Worker to join their CMHT later life team at Etrop Court. If you are looking to develop your NHS experience and provide social care with the support of working within a multidisciplinary team- then please do apply!
Within this role, you will provide a person centred, statutory social work service for a caseload of patients in accordance with relevant policies and procedures. You will be assessing and planning support plans to meet individual needs as well as working with partners to safeguard service users.
Duties- Conduct assessments to determine clients' needs and develop individual care plans- Provide counselling and support to individuals and families facing challenges- Liaise with external agencies and professionals to coordinate services for clients- Advocate for clients' rights and access to resources- Maintain accurate records of client interactions and progress
Requirements- A recognised qualification in Social Work and HCPC registration- Experience working with children is essential- Excellent communication and interpersonal skills- Ability to work effectively in a multidisciplinary team- Strong organisational skills and attention to detail
