Job Summary:

My client is seeking a dedicated and experienced Social Worker Level 3 to join their Adult Social Care team. The successful candidate will be responsible for assessing, planning, and implementing support for adults with varying needs, ensuring they receive the highest quality of care and support.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals’ needs, including physical, emotional, and social factors.
  • Develop and implement person-centered care plans in collaboration with clients and their families.
  • Advocate for clients’ rights and ensure their voices are heard in the decision-making process.
  • Provide support and guidance to clients facing complex challenges, including mental health issues, disabilities, and social isolation.
  • Collaborate with multi-disciplinary teams, including health professionals, to ensure integrated service delivery.
  • Monitor and review care plans regularly, making adjustments as necessary to meet changing needs.
  • Maintain accurate and up-to-date case records in accordance with legal and organisational standards.
  • Participate in supervision, training, and professional development to enhance skills and knowledge.
  • Contribute to the development of policies and procedures related to adult social care services.

Qualifications:

  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • A minimum of [X years] experience working in adult social care.
  • Knowledge of relevant legislation, frameworks, and best practices in social work.
