Job Title: Adult Social Worker (Hospital Discharge)

Location: East London

Position Type: Full-time (5 days/week)

Pay Rate: Up to £32 per hour

Are you an experienced Adult Social Worker with hospital discharge experience and a background in multi-agency work? We are recruiting qualified Social Workers for an authority in East London.

Responsibilities:

• Care Act assessments
• Arranging discharge support
• Mental Capacity Act knowledge and experience
• Report writing
• Caseload management
• Essential visits

Minimum Requirements:

• Social Work England registration
• Social Work Qualification
