Position: Social Worker – Learning Disabilities Team

Location: Hackney, London

Hourly Rate: £32.00 per hour

Employment Type: Ongoing, full-time (36 hours per week)

Key Responsibilities:

• Conduct thorough assessments in accordance with the Care Act (2014) and the Mental Capacity Act (2005).
• Engage in Court of Protection work, advocating for the best interests of service users.
• Develop and coordinate personalised support plans for adults aged 18 and over, requiring ongoing care and reviews.
• Collaborate with a multi-disciplinary team, including Nurses, Psychologists, Psychiatrists, Occupational Therapists, Behavioural Analysts, and Speech and Language Therapists to deliver comprehensive care.
• Participate in multidisciplinary meetings and contribute to the continuous development of service provision.
• Ensure the safeguarding of vulnerable adults and maintain accurate, up-to-date records and documentation.
